    Abstract “Policy-deficits are the most important factor contributing to the ongoing spread, and associated dynamics, of the HIV/AIDS pandemic and its numerous sub-epidemics around the world. Drawing upon ten years experience, observation, and evaluation of AIDS policies, both the general design and core elements of an effective AIDS policy, as well as typical policy impediments, are outlined. Successful AIDS policies follow a strategy of social learning by inclusion and cooperation, are decentralized, and incorporate the motivation and competencies of those affected. Key elements of such a strategy are timely risk perception and assessment; priority to primary prevention; and the formulation of a coherent policy which is integrated into the more general public health programs.”

    Abstract “This paper describes, discusses and analyses the social consequences of AIDS in Namibia from an actor’s perspective. A qualitative approach was chosen for this purpose. Interviews were conducted with HIV-infected and AIDS patients and their families in order to trace what happens at the level of individuals, their households and families when HIV infection, AIDS or death are to be overcome. The study was conducted in an urban context in Windhoek-Katutura and in a rural situation in north central Namibia (Ovamboland). It is based on five months of field research, during which almost 80 interviews were collected and analysed in the study.”

    Abstract “During the entire 19th century cholera was a disease of the poor. The victims of the last European outbreak of cholera in Hamburg in 1892 were almost exclusively workers. Both contemporaries and historians mainly blamed the lack of hygiene standards, which they even interpreted as fatalism of death. Using the Hamburg cholera epidemic as an example, the article explores the subject dimension of the disease and the medical practice of workers. It focuses on an agent-centred perspective in order to broaden the understanding of the excess mortality among working class population by referring to workers’ autobiographical texts. The analysis of these memoirs shows that the workers’ reaction to the disease was geared to health protection and based on traditional practices instead of academic medicine. Representations of death are particularly important in this respect; they can explain the alleged lack of concern and the indifference. The excess mortality among workers during the cholera epidemic cannot be attributed to a fatalism of death but a number of reasons caused by structural and institutional inequalities as well as milieu-specific attitudes, which found expression in workers’ reservations towards academic medicine and in specific concepts of death.” [ABSTRACT FROM AUTHOR]

However, the stigmatization of AIDS patients creates an immensely high inhibition threshold for dealing with the disease aggressively – experts therefore believe that the number of undetected cases of infected persons is enormously high.     Abstract “The course of the HIV epidemic in Germany can be modelled by back calculation until the beginning of the nineties. The recent course of the epidemic can only be derived from surveillance data of newly diagnosed HIV infections in conjunction with other data sources. Based on these surveillance data HIV incidence in Germany can be estimated to have been stable with 2000 to 2500 new infections per year since the early nineties, after having peaked in the early eighties. The most affected group are men who have sex with men followed by persons infected by heterosexual contact and migrants from high prevalence countries. The number of intravenous drug users has declined over the years and is now in fourth place. There are indications that increased risk behaviour and rising numbers of other sexually transmitted infections together with a change towards later initiation of antiretroviral therapy has led to an increase in new HIV infections in Germany in recent years. An improvement of the epidemiological surveillance for “indicator” STIs in combination with the assessment of risk behaviours in high risk groups would be desirable steps towards a second generation surveillance in Germany.”

    Abstract “The SARS lung disease that broke out in China in spring 2003 and the efforts of the Beijing leadership for months to keep the course, dimension and number of victims of the epidemic secret raise the question of political reforms in the Far Eastern country. When Beijing admitted its first mistakes after 22 weeks of secrecy and the first heads rolled, including those of the capital mayor and the health minister, the extent of the consequential damage caused by Chinese misconduct was also revealed on the global stage – the flipside of the mobility and interdependence prevailing in the networked world had revealed itself with alarming clarity. Moreover, the instrumentalisation of the catastrophe by Jiang Zemin, who resigned at the 16th Party Congress of the Chinese Communist Party, and his successors of the ‘Fourth Generation’, as well as the end of information policy tolerance, which came with the abatement of the epidemic, made it clear how far the country is from a ‘Chinese glasnost’ or even the collapse of communist rule. Certainly – SARS has gnawed at the very foundations of the system and damaged its international reputation. However, thanks in part to the stable economic growth in the current China, the end of the system is far from in sight.”

    Abstract “The Ebola fever epidemic in the Democratic Republic of Congo illustrates the urgency of developing cooperation between security, health and development actors. As the disease is spreading, the World Health Organization (WHO) has declared an international health emergency. In crisis situations, the links between health and security are highly complex. Which population groups and which disease are perceived as an alleged health risk, and for what reasons, is a normative question for donor countries. It has political consequences above all for affected developing countries. Where health and safety are common goals, it is not enough to contain only infectious diseases in developing countries. Rather, crisis-resistant, functional and accessible health systems must be established there. This promotes the implementation of the human right to health, creates confidence in state structures and takes into account the security interests of other states. The German Government could advocate a policy in the United Nations Security Council (UN Security Council) under the narrative “stability through health”.”

    Abstract “In crises, existing institutional arrangements, power structures and the interpretation of problems are questioned and decision-makers come under time, action and legitimation pressure. Against the empirical background of the EHEC epidemic in Germany, opportunities and limitations of institutional change through crisis experiences are examined. The case questions the (transformational) power of crises. After all, a central arena of conflict in the policy field of consumer protection cannot be resolved by political decision-makers. Rather, despite the existence of specific crisis conditions, learning is inhibited by existing institutional structures. The decision on the institutionalization of the crisis task force thus follows the limited logic of politics and represents a political innovation only to a very limited extent.”

    Abstract “This doctoral dissertation presents Jewish narrative reaction patterns connected with the Great Plague (1348-49) and the persecutions of the European Jewish communities, which happened during this time. The author examines Ashkenazi founding legends, pijutim, memor books, books of customs, family genealogies, ethical testaments, medical tracts, pijut commentaries, historical songs, fairy tales and crystallizes various motifs of the Jewish plague narratives. The stories about alleged Jewish resistance, forced baptism and self-sacrifice, or about miraculous Jewish plague immunity are found in the fragmentary and geographically dispersed sources centuries after the violent disruption of the Ashkenazi Jewish community. A broader view on Jewish narrative coping with the collective trauma is provided by the narrative parallels to the Jewish catastrophe of the 20th century, the reflection on the increased scholarly interest in post-Shoah medieval persecution, but also by an analysis of the biblical semantic field of disaster.”

    Abstract “In earlier centuries, epidemics and infectious diseases not infrequently affected the smooth flow of postal traffic. As a file from the central archives in Regensburg, which begins in 1607, entitled “The Institutions for Contagions, Infectious Diseases and Horse Diseases on the Imperial Postal System”, illustrates, the Imperial Post Office of Thurn und Taxis was also affected by the resulting restrictions. For example, in the course of the plague epidemic in southern France in 1720, Emperor Charles VI ordered that the Imperial Post Office of Thurn und Taxis had to clean all letters and parcels from France, Switzerland and Graubünden by “außessigung und außräucherung”. In addition, the taxis’ postal authority adopted the recipe and the associated treatment regulations for the fumigation of letters from the Prussian “New Postal Order” of 1712 in 1770/1771.”

    Abstract “Diseases are threats to society – even today. In the 20th century, the “Spanish flu” swept through the world, raging venereal diseases such as syphilis and AIDS, Ebola, SARS, avian or swine flu stoked fears among Europeans. The return of old familiar diseases or the emergence of “new epidemics” called into question social security concepts just as much as the effectiveness of government measures. They marked the narrow limits of political planning utopias and curbed modern euphoria about progress. The authors of the volume set out to trace the social and cultural history of epidemics in the “long 20th century”. They trace the negotiations of collective threats and ideas of security, they explore educational, preventive and control measures and ask about the connection between epidemics and social orders. This search for clues operates on different levels. With their biographical, regional, transnational and comparative approaches, the essays therefore open up new insights into European social and cultural history. For the first time, the volume offers a well-founded overview of the epidemic history of modernity and postmodernity and of the change in social fears, ideas of security and order in the “age of extremes”.”

    Abstract “In the 1980s it was still the custom in some parts of the canton of Lucerne to carry a picture with the Sacred Heart of Jesus as protection against epidemic diseases. Similar religious protection symbols were already in use in the 19th century, when “Asian” cholera spread from India via Russia to Europe, frightening the population, especially in large cities. Robert Koch’s research showed that the new living conditions in 19th century Europe favoured the spread of the disease. Certain areas of Lucerne that were poorly developed in terms of hygiene were also regarded as sources of danger. Although the city was spared a cholera epidemic in all but a few cases, attempts were made to protect against the disease with medicines, but also with religious talismans such as the two amulets from the collection of the Klösterli Museum in Wyher Castle, which are described in more detail here.”

    Abstract “Many diseases lead to social exclusion – but one is even used as a metaphor for isolation: leprosy. This infectious disease provoked by Mycobacterium leprae may – if not treated in early stage – cause strongly stigmatizing alterations of the face and extremities. Therefore, social consequences of leprosy should be considered as important as the physical condition. The isolation of leprosy patients was already stipulated in the Old Testament – in Brazil it was dictated by law until 1962. Some of the former leprosy colonies are still inhabited today. One of these institutions is the “Center of Living together” (CCAD) inaugurated in 1928 in the Brazilian state Ceará, although the residents are free to leave the center since 1981. So far, a structured investigation on life circumstances and social exclusion of the inhabitants of former leprosy colonies in Brazil does not exist. This study with interdisciplinary approach explores life situation and perceived social exclusion of the persons affected by leprosy living in the former leprosy colony by use of quantitative and qualitative methods. To characterize the inhabitants of the CCAD, socio-demographic questionnaires and clinical documentation sheets were applied. Social restriction was measured with the Participation Scale (P-Scale). Relevant life aspects and conflicts were grasped in Focus Group Discussions (FGD). The 90 former leprosy patients in the CCAD were by far older than the 86 non-leprosy-affected residents (median: 64.5 years vs. 16 years, p&lt;0.001). Two third of the former patients lived in little houses and one third in the nursery homes on the grounds of the CCAD. At the time of the study, there was no active case of leprosy in the CCAD. Questionnaire data was obtained from 77 out of the 90 former leprosy patients. As most of them received a state pension, they were not considered poor compared to the population of Ceará. One third of the interviewees had no contact with family members outside the CCAD. Visible physical alterations were documented in 72.7% of the study participants. The P-Scale showed a significant level of social participation restriction in more than half of the persons. The main problems were found in the areas of work, mobility and social activities. The score of the P-Scale (P-Index) was significantly correlated with the severity of physical alterations (p = 0.001), residency in a nursery unit (p = 0.001), time of diagnosis before 1982 (p = 0.002) and receiving no visitors (p = 0.004). Age, sex, income, having a partner and time of residency in the CCAD were not significantly correlated with social restriction. The 17 participants of the FGD discussed motives for living in the CCAD, freedom, every-day life, stigmatization/prejudice and coping with the consequences of the disease. The CCAD was seen as a safe and quiet place by some, while others criticized boredom and expressed the wish for more activity and contact with persons from outside. On one hand, free housing, pension and medical care were named as advantages compared to the “normal” population, on the other hand the participants reported experiences of prejudice and social discrimination. Emotional reactions to humiliation in society included anger, resignation and despair. The interviews showed rejection of the administration personnel with concurrent dependence in the sense of a paternalistic relationship. Coping with stigmatization in public ranged from deception or social withdrawal to open, self-assured behavior. After having been excluded from original family structures, many inhabitants founded new families inside the CCAD. The strong identification with the institution and the clear categories of “us inside” and “those outside” shows that reintegration into society has not been fully successful yet. Rehabilitation measures should be implemented especially in the areas work, mobility and social activities. The feeling of the residents to be respected in the closer community reveals a good potential for self-organization that should be supported in the sense of empowerment. Already existing structures like MORHAN (Movement for the reintegration of leprosy-affected persons) and the regular residents´ meetings could serve as a base for reintegration. On the whole, the study shows that despite cure leprosy is a dominant life theme for the residents of the CCAD. The disease not only changed their biography and shaped their identity but also still causes restriction in social life and is the linking element of the closer community. In sense of the definition of health by the WHO, social rehabilitation of the inhabitants of former leprosy colonies must be seen as an important part of therapy.”

    Abstract “In the middle of the 14th century, the Black Death led to mass deaths in Westphalia and the Rhineland – but even after that, the area was repeatedly hit by devastating epidemics. On the basis of contemporary written testimonies, Kay Peter Jankrift uses the example of eleven cities from Aachen via Cologne and Münster to Minden to show how secular and spiritual authorities as well as the city population dealt with the permanent danger: What mechanisms were set in motion when an epidemic broke out? What strategies did the city fathers develop to contain the “Great Death” over time? And to what extent did the regional exchange of information work? Jankrift focuses in particular on the role of the healers and the factors responsible for the spread of dangerous infectious diseases. An assessment of the significance of the different types of sources for the reconstruction of the medieval and early modern epidemic completes the volume.”
